Sometimes in patients with Extra Capsular Extension (pT3 Cancer) it is not possible to achieve a negative margin. See Figure 5. On occasions this may lead to a PSA recurrence after the surgery. However, even when there is a positive margin, many patients are cured of their cancer with no evidence of a return of a detectable PSA later.
